Compensatory Hypoventilation in Metabolic 296 JAVAHERI

چکیده
Although hyperventilation is a well-known compensatory mechanism in metabolic acidosis, compensatory hypoventilation has been inconsistent and confroversial in metabolic alkalosis. Six healthy subjects were studied under baseline conditions and during steady-state metabolic acidosis (seven episodes) and alkalosis (14 episodes). Minute ventilation (yE) fell in metabolic alkalosis and rose in metabolic acidosis. These changes in ventilation were entirely due to reduction and elevation of tidal volume (VT) respectively, while respiratory frequency (f) remained unchanged. Alveolar ventilation fell

A case of Cushing's syndrome associated with chronic respiratory failure due to metabolic alkalosis.
A case of Cushing's syndrome associated with chronic respiratory failure is presented. Although arterial blood gas analysis showed severe metabolic alkalosis, hypoxemia and mild hypercapnia, the patient had no evidence of pulmonary disease or neuromuscular disorder. Voluntary hyperventilation and inhalation of 100% oxygen (O2) revealed normalized arterial oxygen tension (PaO2). Following the re...
متن کاملHydrochloric acid in the correction of metabolic alkalosis.
Intravenous infusion of hydrochloric acid was used as a safe, effective, and quantitative method for correction of metabolic alkalosis in two patients. The first shows the risks of intravenously administered ammonium chloride, the currently available alternative to hydrochloric acid therapy. The second shows the efficacy of intravenously administered hydrochloric acid. While breathing spontaneo...
متن کاملMathematical equivalent of metabolic alkalosis part of the Goldberg acid-base map.
To the Editor:—Metabolic alkalemia is the most common acid–base disturbance among patients in the hospital. Because the disorder is associated with high mortality and morbidity, its prompt diagnosis and treatment is feasible. The expected physiologic compensation for a primary increase in serum bicarbonate concentration ([HCO3 ]), i.e., metabolic alkalosis, is hypoventilation that increases par...
متن کاملDefining obesity hypoventilation syndrome.
The definition of obesity hypoventilation syndrome (OHS) proposed by Hart et al raises two questions that we have tried to answer briefly in this letter. First, previous definitions of OHS included obstructive sleep apnoea (OSA); Hart et al have excluded it. Why? OSA is present in most patients with OHS and is partly responsible for the hypercapnia. We believe that it is useful to include it in...
متن کاملSleep-Disordered Breathing in Patients with Heart Failure
Breathing disturbances during sleep play a significant role in patients with cardiac diseases due to their high prevalence and impact on outcome. Obstructive sleep apnea (OSA) is a major risk factor of arterial hypertension and is associated with atrial fibrillation. A majority of heart failure (HF) patients suffer from OSA or central sleep apnoea (CSA), both associated with impaired prognosis....
